We require the presence of a parent at check-ups for all children under the age of 18. We feel it is very important that we communicate with and counsel both the teenager and the parent at these yearly check-ups. Please bring a completed copy of the NJ Pre-Participation Physical Exam form (Part A) for athletes. A copy is available and may be printed under "Important Office Notices" to the left.
Prospective patients and expectant parents feel free to call the office to schedule a complimentary appointment to meet with one of our doctors. We will introduce you to our practice and answer any questions you may have.
